You will receive a package in the mail that contains the vial with detailed instructions on how to complete the test. You must not eat, drink, smoke, or chew gum within 30 min of doing the test. In brief, you must collect a saliva sample by spitting into the tube up to the line indicated on the tube. You then close the vial until you hear a click, which signifies that the reagent has been released. You replace the lid of the tube with the smaller lid provided, then shake the vial for 5 seconds. Detailed testing instructions can also be found here:

No. The test only requires a very small amount of saliva. The amount is indicated by a line on the vial. As a reminder, you must not eat, drink, smoke, or chew gum within 30 min of doing the test.
The biggest mistake you can make is not filling out the online form (here) that links your saliva sample to your medical record AND not writing your full name and date of birth on the vial itself with a Sharpie.
When you fill out the online form (here), you will be asked for some identifying information, as well as to scan the barcode on the vial of your test. This is a 14 digit number. Your phone's camera will automatically detect this barcode. If you have issues scanning this barcode, you may enter it in manually.
